A company is due to pay £75,000 after one of its forklift truck overturned in an accident, killing the driver.
Welwood Roofing Services has been fined £50,000 plus costs due to the health and safety breaches which led to the incident.
A second company, Building Research Establishment, is being fined £17,500 plus £12,000 in costs for its role in the accident.
The employee was killed while he was manoeuvring a forklift truck he was not trained to use in order to help him dismantle a fire test rig.
Alison Ashworth, a Health and Safety Executive inspector, says she hopes employers will learn to improve their policies after this case.
She describes the death as avoidable if the businesses had taken sufficient precautions in confirming their workers' training.
"The companies involved [should have] carried out checks to confirm whether or not he knew of the dangers of forklift trucks and how to drive them safely," she asserts.
Earlier this month, NYK Logistics was fined £20,000 after a forklift truck accident which cost one of its workers her leg.
